HOW DO I MAKE AN APPOINTMENT?
Please call our office at (972) 438-3800 to make an appointment. We schedule all visits by appointment only on a first-come-first-serve basis. If you are unable to keep your appointment, we appreciate at least 24 hours notice. However, we do understand that illness or an occasional emergency may arise in which you may have to cancel your appointment closer to your scheduled time. If you are unable to attend your therapy session, please call us as soon as possible to reschedule your appointment.

WHAT SHOULD I BRING TO MY FIRST APPOINTMENT?
On your first visit, please bring the following items with you:
- Your prescription for physical therapy from your referring physician
- A government issued photo ID (such as a driver’s license)
- Your insurance cards
- Appropriate attire (workout clothes)
- If you are referred by a physician outside of our clinic, please bring your physician notes (office visit notes pertaining to your injury, x-ray’s, surgery notes and MRI reports) with you to your first appointment or have this information faxed to our office at (972) 438-3996. Please make sure that the paperwork indicates that you have an appointment scheduled.
- Your new patient paperwork completed. If you are unable to complete this prior to your first visit, please arrive at least 30 minutes early to complete your paperwork. You may download your paperwork on our website.
Note: Patients who are minors (under 18 years of age) must be accompanied by a parent or guardian for their first visit.

WHAT IS PHYSICAL THERAPY?
Physical therapy helps patients return to maximum physical function following injury, surgery or disease. Physical therapy is practice by professionally trained and licensed physical therapists under the referral of a physical. WHAT SHOULD I WEAR TO PHYSICAL THERAPY?
You should wear comfortable clothing or workout clothes (shorts, t-shirt and tennis shoes). Your clothing should allow you to easily access your injured area for treatment by your therapist. We do have a changing room if you need to change into appropriate clothing prior to your appointment. Please make sure to arrive early if you need to change your clothing.

WHAT WILL I DO DURING PHYSICAL THERAPY?
On your first visit, your therapist will perform a thorough initial examination in which your therapist will review your physician’s recommendations, ask questions about your injury, develop an effective treatment plan that is designed just for you, discuss his/her findings with you and answer any questions you may have. The treatment plan that your therapist customizes for you will depend upon your diagnosis. Your treatment may include hands-on therapy, a customized exercise program to complete each visit, a custom home exercise program and physical agents (such as ice, heat, ultrasound and/or electrical stimulation).

HOW LONG ARE MY APPOINTMENTS?
Your fist appointment is typically 45 minutes to one hour; possibly an hour and a half depending on your injury. You need to arrive at least 30 minutes early for your initial visit to fill out your new patient paperwork. You may stop by our office in advance to pick up your paperwork or download it on our website. (if you fill out the paperwork in advance, please arrive at least 15 minutes early). Each follow up appointment usually takes 30 minutes to 1 hour depending on the treatment program your therapist has customized for you.

DOES OUR FACILITY ACCEPT INSURANCE?
Yes. We accept most major insurance companies and treat most Worker’s Compensation injuries. To determine if we accept your insurance, please contact our office. As a courtesy to our patients, we will verify your coverage and benefits information with your insurance and file your claims to your insurance company on your behalf.

HOW OFTEN WILL I NEED TO COME TO PHYSICAL THERAPY?
The frequency and duration of your therapy treatment will depend upon your physician’s prescription and the plan of care developed by your physical therapist. Generally patients require treatment 2-3 times per week. The duration will depend upon the extent of your injury and how you are progressing throughout your treatment sessions. Your physical therapist will discuss this at your initial examination and periodically as you progress.

WHAT WILL PHYSICAL THERAPY COST ME?
The amount you pay each visit will be determined by the benefits of your insurance plan, including your deductible, coinsurance and/or copayment. Payment is due at the time of service. To determine your insurance benefits, you may contact your insurance company or contact our office to verify your insurance.

- Cancellations: Please give at least 24 hours notice when possible; the answering machine is on 24 hours a day.
- Punctuality: Please be on time. If you are more than 30 minutes late, you may be asked to reschedule your appointment. Please contact our office if possible to notify us that you will be late. We try to take our patients back within 5 minutes of their scheduled time. You need to arrive at least 30 minutes early for your initial visit to fill out your new patient paperwork. You may stop by our office in advance to pick up your paperwork or download it on our website. (if you have this paperwork already, please arrive at least 15 minutes early).
- Scheduling: It is recommended that you schedule multiple appointments in advance after your first visit. We will allow you to schedule 1 week in advance. If you don’t schedule your appointments in advance, it may be more difficult to give you the times that you desire.
