What
is your financial policy?
Payment is accepted in full at the time of
service unless other arrangements have been made. Cash, check,
and money order, VISA, MasterCard or American Express, may make
payment.
Our fees may vary with the time spent with
you as well as the complexity of the evaluation or surgical
procedure. Please always feel free to discuss your fee before
your visit or prior to any surgical procedure. Our established
fees are usually in the middle range in comparison to other
orthopedic surgeons.
A fee will be charged for any unusually
long or complex insurance form submitted to us for completion.
Legal forms or reports completed on your behalf usually
necessitate a charge, which will vary according to the amount of
time involved in their preparation.
What
insurance do you accept?

practitioners accepts.
What is your
billing policy?
We are pleased to bill your insurance for
you. Please be prepared with your policy numbers, addresses and
any other information we may need. Feel free to contact our
billing department to ask any questions before your visit. Their
number is
(972) 438-4351.
Please understand that our billing service
is offered as a courtesy to you. It does not obligate us to wait
indefinitely for payment by your carrier.
How do I get a
refill on my prescription?
Whenever possible, prescription renewals
should be called in two days before your supply is exhausted.
Please have the telephone number of your pharmacy available.
What are
your office hours and appointment policies?
Monday through Friday from 8:00 a.m.- 4:30
p.m.
It is often helpful to our patients to
think about the specific concerns they may have, before they
come in. And, make a list of questions for the doctor to answer
during the examination.
If you require x-rays, the receptionist
will ask you to come into the office 15 or 30 minutes early so
that they may be done before you see the doctor.
Routine appointments should be made well
in advance. Please call either the day before, or the morning of
your appointment to verify your appointment time.
If you are unable to keep your appointment
or are going to be late, please call as soon as possible. This
courtesy allows us to be of service to other patients. A charge
will be made for repeated broken appointments.
What should
I bring to my appointment?
Please bring your insurance card,
referral if required by your insurer, test results or x-ray film
and MRI film, a list of mediations you presently take, and
workers compensation authorization if needed.
If another physician has referred you
here, please see that he or she sends your records. The
physician who referred you will receive a letter discussing your
problem and our treatment plan. They will be kept informed of
your progress when indicated.
What is
you policy on Industrial Injuries?
It is your responsibility to file the
appropriate claim with your employer. Also, if your employer has
not authorized your visit they may not be obligated to pay for
it.
A report of your progress is always sent
to the industrial insurance carrier.
Does your
practice offer credit?
Our office does not usually offer credit
privileges, however, if you need to make financial arrangements
for your account please contact the Billing Supervisor for
assistance. It is our policy to work with you in any way
possible should problems arise.
What is
your policy on Emergency Care?
The office answering service can contact
one of our doctors 24 hours a day, 7 days a week. The doctors
take turns being "on call" for emergencies so you may
not get your regular physician in an emergency.
What are your
practices on x-rays and laboratory studies?
X-rays are indispensable to the practice
of orthopedic surgery. Your physician will try to keep the
amount of x-ray exposure to a minimum. If you have any questions
about x-rays being taken or your degree of exposure, please ask
the doctor or the x-ray technician.
If you may be pregnant, please inform the
doctor and the x-ray technician.
Laboratory studies are done at outside
facilities and may be done at the hospital or another
laboratory. Many tests require that you fast the night before
and then have the test done in the morning before breakfast.
Our doctors usually will call you with the
test results only if they are abnormal.
What are you
practices on medications and surgery?
If you have any questions about the use or
side effects of your medication, please ask your physician.
Always discontinue a medication that makes you ill - and call
the doctor. Please give our office at least 24 hour notice in
advance of needing a prescription refill. No medications will be
refilled over a weekend or holiday.
Please have a clear understanding of what
surgical procedure your physician is going to do and why - prior
to surgery. Do not be embarrassed to ask for a second opinion or
another consultation.
Anesthesia and hospital fees are not under
the control of this office and may be substantial.
What is
your policy on disability forms?
We realize how important these forms are
to our patients. We want to make sure these forms are done in a
timely manner. We cannot do this without your help. Please allow
us seven working days to complete these forms.
What
is your policy on Doctor-Patient relationship?
Open communication and trust will speed
your healing process and make your visits here more enjoyable.
If you hare frightened or uncomfortable with your doctor, say
so. Part of the healing process is the caring and the education
that goes on in dealing with a disease. It is always best to
continue treatments with a physician with whom you feel
comfortable.
Your records belong to Irving Orthopedics
& Sports Medicine, but no information will be released
without your permission unless this is required by law, as in an
industrial injury, assault, etc.
